Tomaz Crochemore was Born from Porto Alegre (RS). He is physician specializing in Intensive Care Medicine, focusing on the field of ​​Coagulation and Hemostasis. Graduated from the School of Medicine and Surgery - Federal University of Rio de Janeiro (UNI RIO) between 1995-2001. First medical residence course in Internal Medicine by Hospital of trauma - Hospital Municipal Hospital Miguel Couto / RJ (2002-2004) and Second medical residence course in Intensiva CAre Medicine by the Hospital Pró Cardiaco / RJ - (2004-2006). He is an expert in in Critical Care Medicine (AMIB-Brasilia 2010). Master in health sciences, by SBIBAE, in the area of ​​hemostasis and thrombosis in a critically ill patient. Currently, PhD project in progress.Since 2005, he has been studying and working in the area of ​​hemostasis and thrombosis, focusing on critical ill patients in the perioperative bleeding management, peripartum hemorrhage, cardiac surgery, trauma and DIC. Pioneer in the creation of the specialized support group for bleeding management in critical patients in 2012, which resulted in the creation of the institutional "Code H" at the Hospital Israelita Albert Einstein in 2017. For more than one year he has been working as a coordinating physician of the ICU in the Hospital Leforte-Morumbi, in São Paulo-SP. Today , he is a corporate manager of the ICUs in the Leforte Network Group, São Paulo. Tomaz Crochemore, natural de Porto Alegre (RS), médico especializado em Terapia Intensiva do adulto, com foco na área de Coagulação e Hemostasia. Formado pela Escola de Medicina e Cirurgia - Universidade federal do Rio de Janeiro (UNI RIO) entre 1995-2001, com residência em Clinica Médica pelo Hospital Geral de trauma ? Hospital Municipal Miguel Couto / RJ (2002-2004) e residência médica em Terapia Intensiva pelo Hospital Pró Cardiaco / RJ - (2004-2006). É especialista titulado pela AMIB em Medicina Intensiva do adulto (Brasilia 2010). Mestre em ciências da saúde, pela SBIBAE, na área de hemostasia e trombose em paciente gravemente enfermo. Atualmente, com projeto de doutorado em andamento. Desde 2005 estuda e trabalha na área de hemostasia e trombose, com foco em pacientes críticos, no manejo do sangramento peroperatório, transplante hepático, hemorragia periparto, cirurgia cardíaca, trauma e CIVD. Nos últimos 13 anos vem aperfeiçoando sua prática clínica com os testes viscoelásticos do sangue, como a Tromboelastometria Rotacional, e mais recentemente Multiplate (HIAE).Trabalha com terapia intensiva desde o último ano de universidade (1999), com experiência no cuidado com o paciente grave e medicina de urgência. Dentre inúmeros hospitais que trabalhou, destacam-se o departamento de pacientes graves do Hospital Israelita Albert Einstein, desde o ano de 2011, na função inicial de medico planstonista da UTI, e nos 3 últimos anos como médico diarista da UTI. Atuou como médico plantonista na área de Telemedicina no HIAE (2012-2015). Pioneiro na criação do grupo de suporte especializado em manejo do sangramento em pacientes críticos no ano de 2012, o que resultou na criação do ?Código H? institucional no Hospital Israelita Albert Einstein em 2017. Há mais de 1 ano é médico coordenador da UTI do Hospital Leforte- Morumbi, em São Paulo-SP. Hoje é médico coordenador corporativo das UTIs da Rede Leforte em São Paulo. Hospitais por onde atuou como médico intensivista incluem a UTI geral do Hospital Pró-cardíaco (RJ: 2004-2010) por 7 anos; na REDE Dor Rio de Janeiro,na UTI geral do Hospital Quinta Dor como médico diarista e rotina da UTI (2002-2008), e ainda médico plantonista da Clínica São Vicente da Gávea- RJ. Em São Paulo, antes de ingressar no Hospital Israelita Albert Einstein passou pelos hospitais São Camilo, Hospital São Luis e Hospital São José.Se mantém atualizado com cursos a cada 2 anos em pacientes Neurocríticos, Emergências Cardiovasculares, Hemodinâmica, Via Aérea Dificil, Sepse, Punção vascular guiada por USG, ECO, e curso de segurança do paciente (CRM). Foi criador e coordenador dos cursos de atualização: ?Distúrbios da coagulação e hemostasia em pacientes graves?
